QuoteCloud's template library includes a Dental Treatment Plan Quote, a Dental Treatment Consent Form and a Treatment Plan Proposal. Open one, add your fee list, stages, alternatives, consent wording and photos, save it as your practice's template, and every plan starts from it. One template conversion is free during the trial.
Four document covers from a fictional practice, Wrenfield Dental Group, behind index tabs that take turns at the front: Helen Gordon's restorative treatment plan, Isla Kowal's orthodontic plan, Ravi Pillai's smile plan and the summer check-up offer. Each started from its own template.

Your insurer Practice's wording
Your insurer decides what it pays back. Ask your insurer what it covers for this treatment: no insurance benefit is included or calculated in this estimate.
Paying for your treatment Practice's wording
Your deposit is paid when you sign. Each later stage is billed by the practice when it is done. If you'd like to spread the cost, ask reception about the payment plan providers we work with: any plan is arranged with that provider.
Valid until Wed 18 Nov 2026. The expiry date is set on the document; after it, the practice sends a new plan.

Consent wording saved once, locked in every template
Save your consent acknowledgements, insurance wording and payment wording in the content library and lock them. Drag the locked block into each template: it arrives exactly as written and nobody can change it plan by plan.
When the wording needs to change, the people allowed to unlock it change it in the library and update the templates that use it.

- Locked blocks stay non-editable in plans

Dental template questions
Does QuoteCloud have a dental treatment plan template?
Yes. QuoteCloud's template library has a Dental Treatment Plan Quote, a Dental Treatment Consent Form and a Treatment Plan Proposal, plus templates that suit cosmetic consultations, referral agreements and quick estimates. Open one, change it to suit your practice and save it as your own template.
Are the templates compliant with dental or health regulations?
The templates are starting points, not legal or regulatory advice. Your practice decides the consent, estimate and insurance wording that meets its own obligations, then saves it in the template. With content locking on the Business plan, that wording can't be changed plan by plan.
Can QuoteCloud convert our existing treatment plan into a template?
Yes. During the trial of Essentials, Business or Enterprise, QuoteCloud converts one of your existing document templates for free. More conversions are a paid add-on, priced per template. Paid plans also include a 45-minute training session to help your team start sending plans.
Can we have different templates for each kind of treatment?
Yes. Most practices keep one template per treatment type, such as restorative, implant, orthodontic, cosmetic and periodontal plans, plus a short template for offers. In a group, assign each team the templates it uses, so each practice starts from the right one.
Can our fee list and codes be built into the template?
Yes. Import your fee list into the product catalogue from a CSV, with your own codes as text, then add the treatments to the template's price table. The codes are typed by your practice: QuoteCloud doesn't connect to an official fee schedule or calculate insurance benefits.
