Patient consent, signed by everyone who needs to
Your consent wording kept fixed, acknowledgements confirmed in the document and the patient, guardian or representative each signing online.

QuoteCloud puts consent inside the estimate or agreement: your wording in a locked block (Business), acknowledgements confirmed in a Forms block (Business), and an eSignature block for each signer, from the patient to a guardian or representative. Signers need no account, can verify with a one-time passcode, and each signed document carries an audit trail and a certificate.
Three devices and three signers on one sample psychology treatment plan from a fictional practice, Kinfield Health. Jordan Reyes, 16, signs in person on the practice's tablet; his mother, Sarah Reyes, signs online on her phone after entering a one-time passcode sent by email; the psychologist, Ines Moreau, signs on her laptop. Each signature draws in turn, then the certificate of authenticity reads three of three signed, with the audit trail attached.

Consent wording your team can't change
Write your consent and acknowledgement wording once, have it approved, and lock the block in the template. Practitioners and reception add the plan and the fees; the consent wording stays exactly as approved in every document.
Each acknowledgement is a Forms block field the signer confirms before signing, so the ticks come back with the signatures, in the same document.

Who signs, and how
Several signers on one document, each with their own signature block. Who needs to consent is your practice's decision; QuoteCloud collects the signatures.
The patient or client
Signs from the email link on a phone or computer, or in person on your tablet. No account needed.
A parent or guardian
Gets their own signature block and signs in their own browser, at home or at the appointment.
A representative
A family member or representative signs beside the client, as Claire did for her mother's care agreement.
The practitioner
Can sign too, as the treating practitioner, from their own device.
The right person, a passcode and a record
Turn on the optional one-time passcode and each signer enters a code sent by email before they can sign. A patient at the desk can sign in person on your tablet instead.
When everyone has signed, the document carries a certificate of authenticity and an audit trail. eSignatures are legally binding and eIDAS compliant, on every plan. For hosting and encryption, see privacy, access and security.

Set up consent and signatures in four steps
Done once per template; every document after that carries the same wording.
- 1
Write and approve
Put your consent and acknowledgement wording into the template, as your practice approved it.
- 2
Lock it
Lock the consent block so your team can't edit it, and make each acknowledgement a Forms block field (Business plan).
- 3
Add the signers
Add the patient and anyone else who must sign, and turn on the email passcode if you want it.
- 4
Sign and keep
Each signer confirms and signs, online or in person. The certificate and audit trail come with the signed document.
Consent and eSignature questions, answered
Can a patient and their guardian sign the same document?
Yes. A QuoteCloud document can have several signers, each with their own signature block, such as the patient, a parent or guardian, a representative and the practitioner. Each signs in their own browser or in person on your device, with no account needed. eSignatures are included on every plan.
Can we stop staff editing our consent wording?
Yes, on the Business plan. Content locking keeps a block's wording fixed in the template, so practitioners change the plan and fees but not the consent text. The Forms block lets the signer confirm each acknowledgement inside the document before signing.
Can a patient sign in person at the clinic?
Yes. A signer can sign in person on your tablet or computer, for example at reception or at the end of an appointment, as well as online from the email link on their own phone or computer.
How do we know the right person signed?
Turn on the optional one-time passcode: the signer enters a code sent to their email before they can sign. Every completed document carries a certificate of authenticity and an audit trail. QuoteCloud does not check identity documents beyond the email passcode.
Does QuoteCloud make our consent legally valid?
QuoteCloud eSignatures are legally binding and eIDAS compliant, but QuoteCloud gives no legal or clinical advice. Whether consent is valid, including capacity and who may consent for someone else, depends on your process and obligations, so check them with your own advisers.
Where are signed consent documents stored?
QuoteCloud states that it is hosted on Amazon Web Services with 256-bit encryption, that personal information is stored on servers in the USA, and that it is GDPR compliant. It does not state HIPAA compliance or a health-record certification. Check your own privacy obligations before you start.
